ABOUT LINKING YOUTH TO NATURAL COMMUNITIES
Linking Youth to Natural Communities (LYNC) is a center-based program for at-risk youth and families. LYNC provides supports and services to help court-involved youth, ages 12 - 17, overcome challenges and remain safely in the community. The program provides a comprehensive array of individualized, strength-based services to target behavior change and recidivism reduction. Staff aim to ensure meaningful connections to community-based and natural supports to promote youth and family success.

The Village was one of the first agencies in the country to provide homes for neglected children. Today, we continue to achieve our mission 'to build a community of strong, healthy families who protect and nurture children' by providing a full range of behavioral health, early childhood and youth development, substance abuse treatment, and support services for children, families and adults in the Greater Hartford region.

Provides limited, crisis management-oriented individual and/or family therapy, as needed to LYNC youth and families while they are awaiting an appointment with qualified provider in their community. Also administer the YLS/CMI when needed and/or bio-psycho-social assessment. Assists in implementing programmatic and agency priorities within the LYNC program.
- Provides direct services to LYNC youth and their families which may include:
- Screening, assessment and engagement
- Formulation and contracting for services or treatment plan
- Implementation of treatment plan, modifying it as appropriate during the course of treatment
- Crisis management-oriented services
- Administering the YLS/CMI as needed and/or a bio-psycho-social assessment
- Discharge planning which may include linkages to community support services
- Clinical documentation required by agency and program policies, HIPAA regulations, and funding sources.
- Provision of service in the office or other identified setting to meet the unique needs of families served.
- Works as a team member with other LYNC staff to respond to youth and families and programmatic needs, including the implementation of agency policies, procedures, and initiatives.
- Participate in all quality assurance activities as required.
- Partners with Supervisor to identify and develop plans for professional growth.
- Engage in strength-based, culturally competent, trauma-informed and gender responsive interactions with youth and families.
- Uphold all program expectations.
- Promote a safe, welcoming environment with a focus on risk reduction that and positive behavior change.
- Partners with other program staff to identify and develop approaches to address clients’ unmet needs or service gaps.
- Contributes to program functioning by assuming responsibility for special tasks or leadership in special projects as appropriate.
- Participates in Clinical Supervisor and management trainings as requested.
- Contributes to agency organizational life by participating in agency wide task forces, committees or problem solving teams as appropriate.
- Contributes to agency visibility and development of community linkages by representing the Village in community groups or committees.
- Attend various trainings and meetings as needed to meet the requirements of the position.
- Acquiring and maintaining the needed skills in evidenced based models that the agency/ program ascribe to.
- Other duties and responsibilities as assigned by management.
